At this time, I do not participate in network with any insurance plans. I encourage you to contact your insurance company and find out if your policy offers out of network reimbursement under your mental health benefits.
I maintain documentation standards as required by most insurance companies, and can provide copies of your treatment plan and treatment progress if requested.
If you choose to seek reimbursement from your insurance company, I will provide you with an invoice containing necessary billing codes and information for the services rendered in each session.
Payment is due at time of service. Check is preferred. Cash and Credit Card also accepted.
In special circumstances, for motivated clients experiencing temporary financial hardship, a sliding scale fee schedule may be offered. This is intended to a temporary arrangement to prevent lapse in treatment for motivated clients. Documentation of financial hardship may be requested and fees will be for a limited period of time.
